Nutrition Services, we provide certified diabetes and nutritional education in both individual and group settings, giving patients valuable information and tools to self-manage their disease.
Specialized Medical Treatment, with our model of care, we improve HbA1c levels and other measures of metabolic health. We provide proactive monitoring of diabetes.
Point-of-Care Laboratory, our on-site laboratory provides the convenience of immediate feedback for direct adjustments in care, thereby optimizing efficiency and enhancing both compliance and effectiveness of therapy results back at the time of the clinic visit for most tests.
Proprietary Data Management, at the core of our model is a sophisticated electronic health record and proprietary software that allows us to collect, measure, manage, and link clinical and financial outcomes for patients, payors, and other providers.
